These isozymes are encoded by five genes: IDH1, IDH2, IDH3A, IDH3B and IDH3G. All are metabolic enzymes expressed by eukaryotic cells that act on the substrate isocitrate, converting it to alpha-ketoglutarate (a-KG) via oxidative decarboxylation. The reactions catalysed by IDH1 and IDH2 are reversible and use NADP+ as an electron acceptor leading to the production of NADPH [35]. IDH1 acts in the cell cytoplasm and peroxisomes whereas IDH2 and IDH3 are found in the mitochondrial matrix. The formation of non-mitochondrial NADPH by IDH1 is thought to be an important mechanism for limiting cellular oxidative damage. NADPH also acts as a reducing agent in lipid biosynthesis [36, 37]. The product of the IDH1 forward reaction, a-KG, is an intermediate in the tricarboxylic acid cycle (TCA) and is also involved in nitrogen transportation, oxidation reactions, and amino acid formation. In conditions of hypoxia, the reverse reaction is favored, in which IDH1 catalyzes the conversion of a-KG to isocitrate which can in turn be converted to acetyl-CoA for lipid metabolism [38, 39]. Additionally, IDH1 regulates glucose-stimulated insulin secretion [40].. The characteristic reduction in bone mass per unit of volume, i.e. volumetric BMD in osteoporosis interferes with the mechanical support properties of the bone. Various measures of BMD correlate strongly with compressive failure loads of vertebrae and can therefore be used as convenient and generally reliable indicators of compressive strength of vertebrae in vivo when measured with tools such as dual energy x-ray absorptiometry (DXA) and quantitative computed tomography (QCT) [31, 65]. Approximately 75-90% of the variance in bone strength is accounted for by apparent BMD [53, 87]. Low BMD is one of the most important risk factors for osteoporotic fracture [13]. However, examination of BMD alone is insufficient to explain the basis for vertebral fracture. Studies have shown differences in prevalence rates for fracture among individuals with comparable BMD [1, 12, 15, 18, 28, 46, 61, 73, 79, 89, 90]. These observations suggest that a single measure of BMD may not be representative of vertebral capacity and/or that other factors may operate independent of BMD measures. For example, risk variation between individuals may lie in the more subtle subregional density differences within the vertebral body. Subregional differences are hidden when whole vertebral body BMD is measured [46].. There were several possible reasons for explaining the combination of increased baPWV and PWDC was associated with LVDD. Increased arterial stiffness could decrease diastolic blood pressure, impair the coronary blood supply, induce cardiac hypertrophy and increase cardiac stiffening [41, 42] and then cause LVDD. The increased PWDC represented the heterogeneous and different atrial conduction which might be secondary to the increased left atrium diameter, volume and strain caused by LVDD [19-21]. In the present study, although baPWV and PWDC had a significant correlation with E/Ea and LVDD, there was no correlation between them, which suggested these two parameters might be complementary in predicting increased E/Ea and LVDD. In fact, our patients with the coexistence of increased baPWV and PWDC had the highest E/Ea and higher prevalence of LVDD. Furthermore, the addition of baPWV and PWDC to a clinical model could significantly improve the adjusted R square in prediction of E/Ea and C statistic and integrated discrimination index in prediction of LVDD. Hence, concurrent consideration of baPWV and PWDC might be useful in identifying the high risk group of elevated E/Ea and LVDD..
